   Yes

2. How often will I have to complete this questionnaire?

   Annually

3. Do all of the trainers in our practise have to complete a questionnaire?

   No, only the lead trainer will have to complete the questionnaire. However we do advise that all the trainers sit together and complete it.

4. Do all of the trainees in the practise need to complete the survey?

   Yes, so long as they are not on sick or maternity leave.
